Enrolling in Health Management or Housing services starts with a short orientation to assess where you are and how we can best help you. You’ll start with a small group session to learn about what services CAP provides to people living with HIV, though individual sessions are available if you prefer. These orientations are typically 30 minutes.
After the orientation, you’ll have a one-on-one appointment for one hour to provide an individualized needs assessment for new and returning clients.
Please arrive early to complete necessary paperwork, or get a head start and fill out CAP’s Intake Form here in advance and bring it with you to your appointment. Please also bring with you a photo ID, proof of income (if possible), verification of your HIV status, and any insurance documentation.
