HIV Medical Case Management

CAP SW Washington provides Medical Case Management to People Living with HIV in Clark, Skamania, Cowlitz, and Wahkiakum counties.  We know that everyone’s story is unique, and we know that people living with HIV are now able to live longer, healthier, fulfilling lives.  We want to help you get there!  CAP’s goal for our Medical Case Management program is to link clients with chronic conditions to a continuum of health and social services systems.   Our medical case managers strive to ensure that clients with varying levels of need receive timely coordinated services to find and maintain access to primary medical care and treatment.  We’ll work with you closely to assess your individual needs, your support system, and we’ll coordinate, evaluate, and advocate for your needs to ensure you’re getting the care you need to meet your goals for wellness and self-sufficiency. 

Objectives of Medical Case Management 

  • Provide linkage to a high quality of care through experienced and trained case managers

  • Gather information to assess and determine each client’s needs

  • Develop and implement a service plan

  • Assist client with retention in care and adherence to treatment

  • Identify and reduce barriers that may keep you from getting the care you need

Medical Case Management can connect you to:

  • Linkage to medical care + treatment

  • Medication adherence support

  • Insurance navigation

  • Assistance applying + maintaining eligibility for state and local benefit programs

  • Developing + implementing a goal plan to support health and wellness

  • Internal and community-based referrals to meet basic needs including:

    • Transportation

    • Housing

    • Food + Nutrition

    • Dental services

    • Mental health

    • Substance use treatment

    • Psychosocial support

If you reside in Washington State and are Living with HIV, you’re eligible for Medical Case Management services.  To enroll in our program and access services, you’ll first need to complete an intake with a Medical Case Manager. Start by calling us or dropping by one of our offices in Vancouver or Longview. We’ll complete a quick 5-minute assessment with you over the phone and connect you with one of our specialized Medical Case Managers for an intake as soon as possible.
