CareLink Early Intervention Services (EIS) strives to:

  • Identify people who have been diagnosed with HIV within the past year who may be struggling with barriers such as untreated mental health issues, active substance use or cultural and language barriers that may interfere with connecting to or accessing HIV medical care and case management services.
  • Work with persons who are living with HIV and struggling with barriers such as untreated mental health disorders or active substance use, who have not seen a medical provider for more then six months.
  • Work with persons being released from the local Justice System and the Oregon Department of Corrections who are reintegrating into the community and may need support in connecting with HIV medical services, Case Management and other supportive services.

The CareLink staff work with clients to identify needs and make referrals to:

  • An HIV service provider who best fits the client's needs, living situation, choices and situations.
  • The Partnership Project Case Management intake team.
  • Cascade AIDS Project and other agencies that provide supportive housing services.
  • Drug and Alcohol Treatment Programs.
  • Mental Health Service Providers and Treatment Programs.

CareLink staff:

  • Provide advocacy and support when encountering difficult situations in connecting and accessing services.
  • Make referrals and connections to other community resources as needed.
  • Are available to accompany, transport and advocate for clients to ease the transition from the CareLink EIS program. This helps clients maintain retention with HIV service providers and other supportive programs.
